Annual Check In - Booking Guidelines

To book your Annual Volunteer Check In:

  • complete the Appointment Booking form below (please complete all the required fields and follow all the prompts),
  • once complete, you will receive a confirmation email, as well as a reminder email 24 hours before your scheduled Volunteer Check-in Appointment

Dates and times are available on a first-come, first-served basis. It is preferred and strongly recommended that your Volunteer Annual Check Ins be done in-person with Hospice Orillia staff members at the Orillia Common Roof, but we can make an exception for a virtual alternative under certain circumstances (if you are feeling unwell – please reschedule as needed, if you are out of town/country, etc). Please continue to book your chosen date/time to reserve it.

 

Due to the mandatory requirement of our Volunteer Annual Check-ins, we ask that you connect with us as soon as possible should you be unable to keep your appointment date/time, as it will need to be rescheduled within a certain timeframe.

 

Please note: This particular method of appointment booking is only available to Volunteers of Hospice Orillia for their Annual Check-ins. It is not intended for client use.
